Class Eligibility
You are a member of the settlement if you are a dentist or dental practice in the U.S. who purchased 3M’s Lava Ultimate Restorative and used the product as a dental crown before June 15, 2015, and at least one of your patients experienced one or more full or partial debonds on or before June 15, 2020.

$250 per debond or reimbursement for out of pocket expenses caused by debond.
